PRESENTATION

Elderly Primary Social Services Presentation

PROGRAM PURPOSE
Improving quality of life assisted person regaining capacity for social reintegration, prevention loneliness, isolation, depression, suicide and the promotion of moral and Christian values.

BENEFICIARIES
There are assisted eight people
- Seven need integral social services
- One partial social services

CRITERIA FOR TAKING
-women over the age of 65 years (exception to the rule making elderly between 60-65 years with major health problems and help depedndente).
-have an insufficient income for daily living
-to be alone, without supporters of I degree
-to be mentally healthy
-to work out with the social workers
-to provide accurate and true informations about the situation

-Activities:

1.Counseling - individual discussion which follows the detection of problems, acceptance and settlement solutions comforme Christian principles.

2. Emotional and spiritual  support-group meetings consist in the elderly, in an opportunity to know each other, to know the problems they face and encourage each other. Discussion will also address various topics that aim problems facing older adults, helping them to correctly report them:
-the problem of solitude
-issue negative emotions: anxiety, fear, depression
-the problem of disease, death, self-image
-love, forgiveness, gossip, etc..

3. Grant spiritual support to their kindly to read the Bible and pray together for them
-Bible study (when health allows)
-participation with the elderly in the Baptist Church No. 1 programs

4. Social activities and spend their free type:
-celebrating birthdays, March 8, Easter, Christmas
-company for people confined to bed and not only
-participation to the elderly metings or other activities organized by the foundation (depending on their health)

5. Outdoor recreation activities:
Around-walks in, park, Botanical Garden
-Output of one days in the forest
-Output in the cafeteria
-Services:
-home visits (1 / 2 per week)
-weekly shopping
-intermediation to physicians and other institutions
-accompanying medical and other institutions
-legal advice
-visits to hospital admission-discharge
-payment of bills
easy-cleaning if necessary
-help materials (food, clothing, footwear, medicines, pampers, household items, cosmetics, house restoration, etc.).

Financial Aid (for food and detergent, paying expenses and medicines) to the extent possible foundation

-monthly shopping at Carrefour and Billa
-distribution of food packages to address beneficiaries
-performing and recording acts of donation, the receptions, the T-Bills

ASSISTANCE DURATION:
Assisting occurs lifelong Elder, closing of intervening (as I reported and above) only in case of death, breach of contract of cooperation or desire expressed by the beneficiary not to be assisted.

THE PLAN OF ACTIVITIES FOR THE YEAR 2011

  1. Development planning activities to assist in program of the elderly year 2011 will be between 1-15 January and is planned assistance activities and topics to be addressed in making such elderly advice: planning a year of work activity - plan to assist and volunteer
  2. Develop necessary working tools: social surveys, questionnaires, assessment files, contracts will be collaboration between the 1-15 January will be included data regarding age, income, living conditions, family structure, network support social, occupation, health, risk assessment / prioritization approach these problems
  3. Acquisition the eldest and identification problems will be on January 15 to 31-After the selection, will complement social surveys that will arise problems facing elderly. They will conclude and sign agreements of collaboration, which will present rights and obligations they have as persons assisted, and duration of assisting them. This will form the group of beneficiaries and will sign cooperation agreements.
  4. Recruitment of volunteers will be in January and in the process, according to the needs of the program. We will contact volunteers who have worked so far in the project and potential volunteers will be informed of the program to assist the old women and existing needs in program that may be covered by their services will be informed of the skills required to be selected, will be given their responsibilities and the procedures for assessing their activity, the volunteers will be selected and we  will end a voluntary agreement wich will present rights and obligations of the volunteer, and the duration and frequency of collaboration, they will be informed of the date on which are expected to begin work.
  5. On 1-15 February, will develop a working strategy for each elderly according to specific needs they have will study the problems faced by each person to be assisted and will establish services that will be granted depending on its needs and possibilities of the foundation. The score will be planning the event.
  6. Individual counseling will be made whenever there is need-advice will be given to crisis situations when the social worker deems it necessary and at the request of the person assisted
  7.  Develop materials to support emotional-spiritual groups will be 1-15 during February-material needs of the beneficiaries will include specific topics: the relationship with neighbors, relatives relationship, self image, wrong attitude and the right attitude in relationships with others, of relating disease, death, relationship with God
  8.  Emotional-spiritual support group and promoting Christian moral values. At home we will keep one person assisted whenever needed-in groups, we will discuss topics chosen according to the needs of older persons, ensuring a warm and a time sufficient for expressing opinions and emotions each of them. It will get: the correct reporting to various problems or situations, knowledge, friendship, relationships, increase the feeling of belonging and esteem, prevent loneliness, depression, isolation and even suicidal thoughts, knowledge and understanding of Christian moral values. In the groups will discuss topics chosen according to the needs of older persons, ensuring a warm and a time for expressing opinions and emotions each of them.
  9.  Depending on the needs F.C. Herald will ensure beneficiaries following services: There will be visits (1-2/week) to address the grantee evaluation difficulties, monthly and weekly shopping (they provide food), material assistance (clothing, food, medicines, household items, diapers , walking frames, cosmetics and cleaning products), financial assistance (depending on needs and possibilities), intermediate for assisting in obtaining other sources or personae institutions, medical offices, clinics, cafeteria, Main, Home Insurance, Commission Assessment of Persons with Disabilities, light housekeeping and cooking (occasionally), accompanied to the doctor for consultation, treatment or surgery, hospital stays, visits to hospital discharge, paying bills (monthly), removing drugs from pharmacies, interior restoration (supported habitat).
  10.  Social and leisure activities will be casual - celebration of birth of beneficiaries, festival days of Easter, Christmas and March 8 to participate in festivities organized by the foundation with these occasions, attending meetings with the elderly. Result: relaxation, joy, enhanced sense of belonging correct understanding of the significance of holidays, prevent loneliness, isolation, depression
  11. Outdoor recreational activities will be organized during May to October-There will be various activities (depending on their health status) output in the cafeteria, a walk in the Botanical Garden, exits the church, walking in the park, check-out day the forest, organizing a camp for 3 days. Results obtained: socialization, relaxation and good mood, happiness, friendship relationships, prevent loneliness, isolation, depression and suicidal thoughts.
  12.  Company is given when needed and in particular for house-is fixed in time we grant (through visits) especially those confined to bed or convalescenta.Timp period in which they listen, we read, we bring fruit or sweets and strive to bring back the joy on their faces. Through this service they were part of: encouragement, consolation, joy, enhanced sense of belonging, eliminating loneliness, information.
  13.  Informing the public about the services it performs charitable foundations Herald whenever needed by: - Organizing conferences, interviews on radio presentation, open discussion, distribute flyers and brochures, media materials. Purpose: information and awareness of population about the existence of foundations and services.
  14. Monitoring volunteers will be made in January-December 2011 - There will be meetings with the volunteers every two months that will evaluate the results obtained in their work is voluntary and will hold weekly telephone. Purpose: To streamline the work of volunteers.
  15. Develop overall work plan for next year will be made in October 2011 - The new year work plan is established who will be assisted and elaborates annual budget. Purpose: to assist the work oranizarea new year.
  16. Taken into account as current beneficiaries and potential beneficiaries: will be done in October-November 2011 - Social workers will move to address those who require assistance, which will apply the criteria for selection of persons involved in the project and on this occasion will achieve and social investigation on the grid and made a preliminary assessment problems they present and future beneficiaries. Purpose: The selection of beneficiaries for retrieval.
  17. Assessment and evaluating the work situation of persons assisted: April-September to December 2011 - On the basis of evaluation tools (evaluation sheet, questionnaire) will determine the extent to work coordinator, volunteers, was effective and how effective was the elderlyr assistance (which will see the evolution or involution of the relationship between them and the supplier-recipient). We get efficient labor elderly support
  18. Related decisions will be assisted where appropriate-In this stage on the final evaluation will take decisions for each person assisted in part. Assisting occurs throughout life but the Elder, for breaches of contract, if the assisted person's death or if you do not want elderly assistance, will ensure the cessation of social care services in this case / termination of the collaboration.
  19.  Conclusion of work will take place on 1-15 December to appropriate cases will be class cases will be the general report of activity and will teach file program director.
